Registration for the 2023-24 school year is now available through the  Infinite Campus Portal. Please login to your portal account to review the current information and make any necessary changes. If you do not have a portal account, you should have received an email on March 17th inviting you to create one. 

TO PARENTS OF FIFTH GRADERS:

• A current physical examination is required for entry into the 6th grade.  Parents must complete the Health History section of the physical form. (physical exam form available in SD54 website)  All children entering sixth grade must show proof of receiving 1 dose of Tdap regardless of the interval since the last DTaP, DT, or Td dose on or after the 11th birthday.
• All children entering sixth grade must show proof of receiving 3 hepatitis B vaccines.
• All students entering sixth grade must show proof of having received two doses of varicella vaccines.
•  All children entering the sixth grade must show proof of having received one dose of meningococcal conjugate vaccine on or after the 11th birthday. 

Please have the Certificate of Child Health Examination (sixth grade physical examination form) completed by the physician and documentation showing proof of required vaccines and return it to the school nurse by the first day of school in  August 2023.  

Sixth grade students who have not had the above requirements completed will be excluded from school in the fall.
