Health Information from the Nurse
Please make sure you return the Child Health Examination to our school nurse. These forms are for ALL Kindergarteners, sixth graders and students new to District 54. The form includes documentation of required immunizations, a completed health history section with guardian signature as well as the physician’s signature.
ALL kindergarten students must have a vision and dental exam. We ask that all medical forms be turned into the school office by the first day of school. If you have any questions, please contact our school nurse Mrs. Christine Kolcz.
